Automated Clearing House (ACH)

To submit an ACH Form, please complete the following steps:

Step 1: Please download the required editable ACH Form:

Step 2: Open with Adobe Acrobat and complete. (If you do not have Adobe Acrobat you can download here)

Step 3: After completion, verify that included information is accurate, then save the document.

Step 4: Submit your completed Automated Clearing House Form by filling out the short form below and attaching the completed claim form, along with supporting documents then click the SUBMIT button.

Required Documents:

  • Bank Statement with account holder number is required.
  • One form of valid photo ID (National ID, Driver’s Permit, Passport)
