To submit an ACH Form, please complete the following steps:
Step 2: Open with Adobe Acrobat and complete. (If you do not have Adobe Acrobat you can download here)
Step 3: After completion, verify that included information is accurate, then save the document.
Step 4: Submit your completed Automated Clearing House Form by filling out the short form below and attaching the completed claim form, along with supporting documents then click the SUBMIT button.
- Bank Statement with account holder number is required.
- One form of valid photo ID (National ID, Driver’s Permit, Passport)
I hereby agree to have my Loan proceeds, or any other payment to my personal bank account by PSCU Credit Union Co-operative Society Limited as stated above and agree to abide by the terms and conditions of this agreement. Failure to submit the correct Bank details may result in a charge by the Commercial Bank. Funds will be applied within 24 to 48 hours of the request.
