To submit an Insurance Claim, please complete the following steps:

Step 3: After completion, verify that included information is accurate, then save the document.

Step 4: Submit your completed Claim Form by filling out the short form below and attaching the completed claim form, along with supporting documents then click the SUBMIT button.

Frequently asked questions

For medical expenses there is no waiting period. However, there is a 3 month wait period for dental and vision claims.

The member has the option of either a cheque payment or direct deposit to their designated bank account.

The health plan has an 80/20 coverage on reasonable and customary charges; 80% is covered by the insurer and 20% is the responsibility of the member.
